Early Pregnancy Scan: When Is It Needed and What Does It Show?

Early Pregnancy Ultrasound — কখন Early Pregnancy Scan করা দরকার?

Finding out that you are pregnant is an exciting moment—but it is also natural to have many questions.

“Baby ঠিক জায়গায় আছে তো?”
“Heartbeat দেখা যাবে?”
“কত সপ্তাহে scan করব?”
“সব pregnancy-তে কি early scan দরকার?”

An early pregnancy ultrasound scan can answer many of these questions and, in selected situations, can help detect problems such as ectopic pregnancy or early pregnancy loss.

It can help the doctor understand the location, development and viability of the pregnancy, depending on how many weeks pregnant you are.

Early Pregnancy Scan

What is an Early Pregnancy Scan?

An early pregnancy scan is an ultrasound examination performed during the first few weeks of pregnancy.

It may be done through the abdomen (transabdominal scan) or, particularly in very early pregnancy, through the vagina (transvaginal ultrasound/TVS).

Because the pregnancy is very small in the early weeks, a transvaginal scan can often provide clearer images of the uterus, pregnancy sac and surrounding pelvic structures.

  • Gestational sac
  • Yolk sac
  • Fetal pole/embryo

When Is an Early Pregnancy Scan Needed?

Not every woman needs an urgent scan immediately after a positive pregnancy test.

The timing depends on the gestational age, symptoms, previous pregnancy history and individual risk factors.

An early scan may be particularly useful when there is:

1. Vaginal bleeding

Pregnancy-এর প্রথম দিকে bleeding হলে scan করা প্রয়োজন হতে পারে to determine the location and viability of the pregnancy.

2. Lower abdominal or pelvic pain

Pain in early pregnancy should not simply be ignored.

An ultrasound may help assess whether the pregnancy is inside the uterus and whether there are findings that raise concern for ectopic pregnancy. (Nice)

3. Previous ectopic pregnancy

A woman with a previous ectopic pregnancy may require early assessment in a subsequent pregnancy.

4. Uncertain dates

If menstrual cycles are irregular or the last menstrual period is uncertain, ultrasound can help establish pregnancy development and subsequently dating.

5. Previous miscarriage or pregnancy complications

Depending on the individual history, your gynaecologist may recommend an early assessment.

6. Fertility treatment or IVF pregnancy

Women who conceive following fertility treatment are commonly followed more closely in early pregnancy, and an early ultrasound may be recommended by the treating doctor.

7. Severe pain, dizziness or other concerning symptoms

These symptoms require prompt medical assessment because an ectopic pregnancy can sometimes become an emergency.

What Can We See on an Early Pregnancy Scan?

What we see depends greatly on how many weeks pregnant you actually are.

This is important.

A scan done too early may not show everything—even when the pregnancy is completely normal.

Therefore, please don’t panic if your doctor says:

“It is still too early. We need to repeat the scan.”

The date of the last menstrual period is not always an accurate indication of the actual developmental stage because ovulation can occur earlier or later than expected. NICE specifically advises not to diagnose viability based only on the LMP date.

Around the Early Weeks

Depending on gestational age, an ultrasound may demonstrate:

Gestational sac

The first visible structure of an intrauterine pregnancy may be the gestational sac.

Yolk sac

The yolk sac can subsequently become visible within the gestational sac.

Embryo/Fetal pole

As pregnancy develops, the embryo becomes visible.

Fetal heartbeat

At an appropriate stage of development, cardiac activity may become visible.

But remember: absence of a heartbeat on a very early scan does not automatically mean miscarriage.

If the pregnancy is very early or measurements are borderline, a repeat scan after an appropriate interval may be necessary before reaching a definite conclusion.

One of the Most Important Questions:

“Is the Pregnancy Inside the Uterus?”

This is one of the major purposes of an early pregnancy assessment.

A pregnancy developing outside the uterus is called an ectopic pregnancy.

Most ectopic pregnancies occur in the fallopian tube.

Ultrasound looks at the uterus as well as the adnexal regions around the ovaries and tubes. Certain findings may suggest or strongly indicate an ectopic pregnancy, but the diagnosis sometimes requires correlation with symptoms, examination and serial β-hCG measurements.

Does Early Pregnancy Scan Confirm That Everything Is Normal?

Not completely.

This is a very important point for patients.

An early scan can provide valuable information about:

  • Location of pregnancy
  • Development at that stage
  • Cardiac activity when visible
  • Uterus and adnexa
  • Possible ectopic pregnancy
  • Possible early pregnancy complications

But it cannot rule out every possible problem in pregnancy.

The detailed first-trimester assessment and subsequent anomaly scan have different purposes.

What If the Scan Shows No Heartbeat?

Don’t panic immediately.

Sometimes the pregnancy is simply too early.

For example, NICE recommends repeat assessment rather than making an immediate diagnosis of miscarriage in certain very early situations where the embryo is small or the gestational sac does not yet show an embryo. (Nice)

Therefore:

“No heartbeat seen today” ≠ automatically “miscarriage”.

Your doctor will consider:

  • Gestational age
  • Crown-rump length
  • Gestational sac measurements
  • Previous scan
  • Symptoms
  • Clinical examination

and may recommend a repeat ultrasound.

Transvaginal Scan — Is It Safe?

Transvaginal ultrasound is commonly used in early pregnancy because it can provide clearer images when the pregnancy is very small.

When clinically indicated, it is an established diagnostic examination for assessing pregnancy location and viability. (Nice)

Your doctor will explain the procedure and answer any concerns you may have.

Early Pregnancy Scan: A Simple Timeline

Approximate stage What may be assessed
Very early pregnancy Pregnancy location and development, depending on gestational age
Around 5–6 weeks Gestational sac/yolk sac and sometimes embryo may become visible
Around 6–7 weeks Embryo and cardiac activity may often be identifiable, depending on dating
Later first trimester More detailed assessment and pregnancy dating

These are approximate ranges, not rigid rules.

Ovulation and implantation can occur at different times, so two women with the same LMP date may not show exactly the same findings on ultrasound.

FAQ — Frequently Asked Questions

1. I have a positive pregnancy test. Should I immediately do an ultrasound?

Not necessarily.

If you have no concerning symptoms, your gynaecologist may advise you about the most useful timing based on your menstrual dates and individual circumstances.

2. I have mild spotting. Should I have a scan?

You should contact your gynaecologist.

Bleeding in early pregnancy does not always mean miscarriage, but assessment may be needed depending on the amount of bleeding, pain, gestational age and pregnancy history.

3. I have abdominal pain in early pregnancy. Is it normal?

Some mild discomfort can occur for many reasons, but significant, persistent or one-sided pain should not be ignored.

Especially if pain is associated with bleeding, dizziness, fainting or shoulder-tip pain, urgent medical assessment is important because ectopic pregnancy must be considered.

4. Can an ultrasound detect ectopic pregnancy?

Ultrasound is an important part of assessing suspected ectopic pregnancy.

However, an ectopic pregnancy cannot always be diagnosed from one scan alone, particularly when the pregnancy is extremely early. Clinical symptoms, ultrasound findings and sometimes serial β-hCG measurements may all be required.

5. If there is no heartbeat on the first scan, does it mean miscarriage?

No—not necessarily.

If the pregnancy is very early, cardiac activity may not yet be visible.

A repeat scan may be required before making a definite diagnosis. (Nice)

6. Is transvaginal ultrasound better than abdominal ultrasound in early pregnancy?

For very early pregnancy assessment, transvaginal ultrasound generally provides clearer images of the uterus and adnexal structures than an abdominal scan.

7. Can an early scan tell me my baby’s gender?

No. An early pregnancy scan is not performed for determining fetal sex.

Its purpose is to assess the pregnancy medically.

8. Does an early scan replace the anomaly scan?

No.

Early pregnancy assessment and the later detailed fetal/anomaly assessment answer different questions.

A normal early scan does not mean that all later structural assessments can be skipped.

When Should You Seek Urgent Medical Attention?

Please seek prompt medical care if early pregnancy is associated with:

  • Severe or increasing abdominal pain
  • One-sided pelvic pain
  • Heavy vaginal bleeding
  • Dizziness or fainting
  • Shoulder-tip pain associated with abdominal symptoms
  • Severe weakness or collapse

These symptoms can sometimes indicate a significant early pregnancy complication, including ectopic pregnancy.

Early Pregnancy Scan — 5 Things Every Pregnant Woman Should Remember

1. Positive pregnancy test does not always mean you need an immediate scan.

2. The correct timing depends on gestational age and your individual circumstances.

3. Early ultrasound helps determine the location and development of the pregnancy.

4. If the pregnancy is very early, everything may not be visible in one scan.

5. Pain or bleeding in early pregnancy should never be ignored.

“Too early to see” মানেই “problem আছে”—এটা নয়।

Final Advice

Every pregnancy is different.

Please don’t compare your ultrasound report with your friend’s or relative’s report.

The same gestational age on paper does not always mean exactly the same biological development.

If you have pain, bleeding, previous ectopic pregnancy, previous miscarriage, fertility treatment or any other concern, discuss the timing of your scan with your gynaecologist.
